The climate crisis is one of the biggest challenges facing our planet and healthcare providers are often on the frontline dealing with its impact on our physical and mental health. However, the healthcare industry is also a major contributor to greenhouse gas (GHG) emissions (some reports put it as high as 4.6% of global GHG emissions.) In our final episode of Transformational Healthcare Leadership we explore the leadership and collaboration needed in the sector to tackle the sustainability challenges with Dr Rana Nabulsi from the United Arab Emirates. Rana is the Head of Operations in Quality-Pathology and Genetics at the Dubai Health Authority with academic and medical credentials in both leadership and science. She is constantly looking to improve healthcare systems, reduce waste and deliver better value for money for all. Eleanor Murray picks up the microphone from Dr Mark O’Brien once again for this episode. She talks to Rana about her experience with benchmarking processes, costs and patient outcomes with other healthcare organisations. But as benchmarking is more challenging with sustainability, where best practice is still very embryonic, Rana says the best scenario is to keep comparing yourself with the previous year to improve performance, and also where possible to look outside your own sector for insights from other industries where waste and emissions might be similar. ‘The enjoyment of the highest attainable standard of health is one of the fundamental rights of every human being without distinction of race, religion, political belief, economic or social condition’. This is a founding principle of the World Health Organisation which was set up 75 years ago. But the reality is that there is a huge disparity in access to healthcare across the world and within countries. New Zealand is one of the many countries where this so-called ‘postcode lottery’ of care is evident. Following an extensive review which highlighted the chronic inequities in the healthcare system, particularly in relation to Maori and Pacific people, the government in April 2021 announced a major overhaul of the public health system. In July 2022 the Pae Ora (Healthy Futures) Act took effect establishing four new entities to replace the previous system which was structured around 20 regional District Health Boards. In episode 5 of Transformational Healthcare Leadership, Dr Mark O’Brien talks to Stephen McKernan who was tasked with leading the New Zealand Health Reform Transition Unit. In their conversation Stephen talks to Mark about the scale of the challenge and what approach he took to the design and delivery of this new system which aims to create a national health system, where the treatment people get is not determined by where they live. In episode 4 of Transformational Healthcare Leadership, Dr Mark O’Brien talks to Annie Flamsteed, the Australian Founder and Executive Chair of iNSPiRETEK. ‘How can we use technology to better support or accelerate access to care to prevent burnout rather than dealing with it once it is already upon us.’ Annie started her career as an elite competitive gymnast but chose to exit the sport in her late teens due, in part, to the incredible pressure placed on high-performance athletes. Her lived experience of the mental health challenges that she and many of her peers face makes her passionate about preventative healthcare; it inspired her to ‘hack the system’ so that data analytics could be used to predict when, or if, someone was potentially going to struggle with mental illness in order to accelerate their access to clinical healthcare. Personal data collection in intrinsic to iNSPiRETEK’s services and Mark talks to Annie about the challenges around security and confidentiality with sensitive data particularly as much of their work revolves around young athletes. She says for her organisation it is really important that they only do business with ethical partners. And as this is a healthcare leadership podcast Annie and Mark had a fascinating discussion about the personal and professional drivers that have shaped her leadership journey so far. Sheer grit and determination and a passionate commitment to the cause have absolutely been key; but also key to spring boarding iNSPiRETEK forward has been the ‘humility to ask for help’ and when it came to raising capital she looked for ‘smart capital’ and found people who provide help along with funding. In episode 3 of Transformational Healthcare Leadership, Dr Mark O’Brien hands the microphone over to his colleague Eleanor Murray, Academic Director of the Oxford Healthcare Leadership Programme. The setting for this episode is India where Eleanor explores the innovative approach to healthcare in the world’s most populous country, with Professor Anupam Sibal. ‘It doesn't always take huge amounts of money to be able to innovate. And I think India has many examples of frugal innovation.’ Professor Anupam Sibal is the Group Director of Medical Services for Apollo Healthcare India, an integrated healthcare delivery system with a strong physical and digital presence spanning the healthcare spectrum and covering all levels of patient care. For centuries innovation has been at the heart of the incredible advances we have seen in medical diagnostics, treatments and cures. But for Anupam what sets India apart from other countries is not just its spirit of innovation but its spirit of ‘frugal innovation’ – always asking how you can get more from that one pound or one dollar. Medical equipment is expensive he explains so you need to ‘sweat those assets’ for maximum return. Investment in healthcare is a long-term strategy for Apollo Healthcare and that means investment in education which is the bedrock of future patient care. But their investment goes beyond the bricks and mortar of their schools and colleges with a strategy to leverage technology for upskilling. And with a workforce of 750 million India will not only be able to cater for the healthcare needs of India but will also be able to help heal the ageing global population. In episode 2 of Transformational Healthcare Leadership, Dr Mark O’Brien talks to Natalie Smith, a specialist in digital transformation in health and community services in Australia. ‘You can't pay someone else to do your exercise and expect to lose weight.’ Natalie’s career has been grounded in and around technology projects in a range of industries. In her early career, she was a systems and data engineer, implementing technology for a range of private and public organisations. More recently Natalie has been specialising in the digital transformation of health and community services. Her roles have included being a partner in Deloitte's risk advisory practice, as well as the chief delivery officer for the state health department in Queensland which is at the forefront of digital health transformation in Australia. She now has a number of significant governance roles in large university, and health and community service organisations in Australia. She is passionate about helping organisations successfully implement their technology projects to deliver better healthcare. In their conversation Mark talks to Natalie about the role of organisational leaders in transformational projects and her theory of ‘transformational friction’ - the forces at play between the existing organisation and the organisation that's changing. In episode 1 of Transformational Healthcare Leadership from Saïd Business School, Dr Mark O’Brien talks to Professor Bruno Holthof, a Visiting Professor of Health Innovation in the Nuffield Department of Medicine at Oxford University and an investment partner of the EQT Health Economics Fund. “My advice is if you really have some important issues to solve, make sure you prioritise and be ruthless in your prioritisation.” Bruno is an incredibly experienced healthcare leader having spent nearly 40 years successfully straddling the scientific and management world. After qualifying in medicine Bruno went on to earn an MBA and then a PhD in Health Economics. He was a partner with McKinsey & Company and served a wide range of healthcare clients, gaining significant expertise in the areas of strategy, organisation and operations. He later moved into an executive position, first as CEO at Antwerp Hospital Network (ZNA), where he transformed ZNA into the most profitable hospital group in Belgium, and then as CEO of Oxford University Hospitals (OUH) where he was involved in the development of the Oxford-AstraZeneca Covid-19 vaccine. He said one of the most important things that he did in both organisations was putting the right leaders in charge of each of the clinical services. Bruno is passionate about innovation in the healthcare industry and sees it as a key driver in improving patient outcomes. He is convinced that in the next 10 years the advances in genomics and artificial intelligence will translate into real benefits for individual patients and entire populations.
